Please enter information about Additional Administrators below. This will allow the designated person to use the credit balance on your account.  If you choose, the Additional Administrator can also make additional charges to your credit card (that will be automatic when there are not sufficient funds to cover the tests being ordered).  Once you create the account, an email will be sent to the Additional Administrator's email address.
